Parent Handbook

This handbook is designed as a parent’s guide to Discovery Place. In the handbook you will find answers to many of the questions and concerns that you may have. We look forward to working with you in caring for and educating your child. We encourage your participation, cooperation, comments, and support.

Discovery Place Staff

Health Care Summary Form

This form tells us a little about your child’s medical history. This should be completed for every child.

Infant Rolling Over Parent Statement

This form should be completed if you allow Discovery Place to leave your child on their stomach while sleeping if they independently roll onto their stomach.

Infant Social Resume

This form tells us a little bit about your infant including their likes, dislikes, habits, and routines. This should be completed for every infant.

Long Term Medication Form

This form should be completed if you need Discovery Place to administer prescription or non-prescription medications for longer than one week. One form per child should be completed, if applicable.

Medication Permission Form

This form should be completed if you need Discovery Place to administer prescription or non-prescription medications for a period of one week or less. One form per child should be completed, if applicable.

Permission Agreement Form

This form will allow Discovery Place to provide simple first-aid treatment to your child when necessary.

Toddler and Up Social Resume

This form tells us a little bit about your child including their likes, dislikes, habits, and routines.
